Dom Care (Homecare)
Our domiciliary care (also referred to as dom care or homecare) is delivered at a time and in a manner which is suitable for the service user and their family. Our support programme is designed around the individual’s strengths and preferences and provides quality of outcomes, spending as much time as needed to fulfil the wishes of the individual.
Our service provision supports individuals who have a physical and/or learning disability, brain injury, mental health, Parkinson’s, dementia or those who have needs arising from being elderly.
Companionship
The UK’s Loneliness Strategy recognises the relationship between loneliness and major health conditions including heart disease, stroke and dementia. With new initiatives, such as social prescribing being brought into communities across the UK, we work on a local level to deliver social connectedness to tackle loneliness. Our companionship service offers social connectedness through booking and attending appointments, supporting to participate in activities of leisure, hobbies and crafts, socialising and engaging with preferred groups including family and accompanying individuals to outings.
Children’s Services
Empress Social Care provides High Level Family Support for children and young people with disabilities and/or complex needs. Our service delivery model is based on intensive, targeted interventions designed to support families who are experiencing significant challenges that may place the child or family at risk of breakdown, harm or escalation in needs.
Community Outreach
Our Community Outreach support is designed to support individuals who live in the family home and require a few hours of support to help maintain or gain their independence. We work with individuals to develop a personalised support plan that is appropriate and suitable to their aspirations and their needs. We consider the goals and priorities we need to focus on to achieve the desired outcomes.
We support individuals to develop and improve life skills. We liaise with the individual’s family, friends and carers as well as other agencies to provide and ensure a seamless service.
Respite Care
The vital service of respite care offers relief to caregivers who support individuals with ongoing health needs, disabilities or aging-related challenges. Our service enables family members and caregivers to take a break, knowing their loved ones are in safe, professional hands. Respite care is delivered in the home providing short-term care tailored to the individual’s needs.
Our support not only benefits caregivers by giving them time to rest and recharge but also supports the wellbeing of the individual by offering continuity of care in a comfortable environment.
Supported Living
Our service is firmly rooted in fostering positive outcomes and cultivating an environment that empowers individuals to attain independence and confidence, ultimately reducing the need for support when it becomes appropriate to do so. Our overarching vision is one of equality, where individuals lead meaningful lives.
