Our Community Outreach support service is designed to support individuals who are usually living in the family home and typically require a few hours of support to help them maintain or gain their independence.

We work with individuals to develop a personalised support plan that is appropriate and suitable to the individual’s needs. We will consider what goals and priorities we need to focus on to achieve the desired outcomes.

We support individuals to develop and improve life skills. We liaise with the individual’s families, friends and carers as well as other agencies to provide and to ensure a seamless service.

Staff are carefully selected and matched according to the preferences and needs of the individual.
